I release and forever discharge Elite Softball, LLC., Elite Baseball, LLC., and/or Maelynn Leber, Hannah Swartz, and its employees, staff, represenatives, affiliates, and all others from any and all responsibilities or liability for injuries or damages resulting from my participation in any activities or use of any equipment at the Elite Baseball/Softball Facility. I do also hereby release all of those mentioned and any others acting upon their behalf from any responsibility or liability for any injury or damages to myself, or activities or my use of equipment in the Elite Baseball/Softball facility. I do also hereby release all of those mentioned and any others acting upon their behalf from any responsibility or liability for any injury or damage to myself (or my child), including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf or in any way arising, out of or connected with my participation in any activities on the property of Elite Baseball/Softball.
You acknowledge that you have carefully read this waiver and release and fully understand that it is a release of liability and assumption of risk. You are aware and agree that by executing this waiver and release, you are giving up your right to bring a legal action or assert a claim against Elite Softball LLC., Maelynn Leber, and Hannah Swartz. You have read and voluntarily signed the waiver and release and further agree that no oral representations, statements, or inducement apart from the foregoing written agreement have been made.
