I verify, to the best of my knowledge, my child is able to participate fully in the Elite Softball program. In case of medical emergency and the event that the parent/guardian cannot be immediately contacted, I hereby give my permission for emergency treatment (i.e. EMT, First Responder, E.D) to be administered to my child. I release Elite Softball, LLC and their employees from all liability for any personal injury, illness, loss or damage to property. I agree to assume liability for any expense*

I authorize all medical and surgical treatment, X-ray, laboratory, anesthesia, and other medical and/or hospital procedures as may be performed or prescribed by the attending physician and/or paramedics for my child and waive my right to informed consent of treatment. This waiver applies only in the event that neither parent/guardian can be reached in the case of an emergency.

I give my permission for Elite Softball, LLC to use any photos/videos taken during the season in which my child may appear in:

Cancellation and Refund Policy: Elite Softball does not offer cash refunds under any circumstances. All cancellations must be submitted via email to elitesoftball@gmail.com. Cancellations made less than (14 days) from the start date of a camp will forfeit their registration. There are no exceptions. *

Cancellations occurring between 14 and 28 days from the start date of an event will receive a 50% credit to a future camp. The credit will be valid for 1 year from the start date of the cancelled camp. Cancellations occurring outside of 28 days from the start date of a camp will receive a full credit towards a future camp.*

Inclement Weather Policy: In the event of inclement weather, Elite Softball, may adjust the schedule, including to the game and showcase schedule, to get in as much as possible. Any changes to the schedule will aim to ensure the maximum amount of showcasing. Regardless of any adjustments made by Elite Softball, all events remain fully non-refundable in the event of inclement weather.*
